<p>113953.2b liquid soap dispenser in the restroom was noted empty. Refill as often as needed.<br />
Towel dispenser in the restroom was noted missing.  Provide towels in an approved dispenser.</p>
<p>Wall in the back storage area was noted dilapidated with holes.  Repair and paint the wall.</p>
<p>Floor drain in the back storage was noted dirty and filthy.  Clean and maintain.</p>
<p>The facility was noted operating without a valid health permit as required by law.  Obtain a valid health permit within 10 days or enforcement action will start which will include closure of the facility.</p>
<p>Provide current health cards for the employees within 5 days.</p>
